If you would like to make a change to your existing membership, please fill out the form below. You will receive an email once the change has been completed.
A FEW THINGS TO NOTE
- All freeze and cancellation requests must be submitted by the 15th of each month to avoid charges for the next month.
- There is a $15 charge each month your membership is frozen.
- Freezes due to injury are free. To request a medical freeze, email email@example.com. You will be asked to provide a doctor's note.
- To freeze an Annual Paid in Full membership, email firstname.lastname@example.org.
